  • Mission Statement: Our goal is to help patients and patients’ caregivers to have sufficient knowledge and resources to manage their medications and medication-related issues, to help identify and screen for early signs or symptoms of chronic diseases, and to establish long-term medical relationships between pharmacists and patients so that medication-related issues can be avoided. These goals will allow patients to remain healthier while also providing cost-effective and safer medical care. 
Our philosophy is that every patient is unique and has different medication needs. We will tailor each patient’s medication regimen to his/her personal needs.
Our clients
Our services are particularly useful for elderly patients (65 and older) who have multiple chronic conditions taking multiple medications. However, our clinical pharmacy service will be available to others regardless of age. We also can work with caregivers and health providers of patients.

The Medicare Modernization Act of 2003 (MMA) under title 42 CFR Part 423, Subpart D, establishes the requirements that Part D sponsors must meet with regard to cost control and quality improvement. These standards, also, are the requirements for MTM providers.

Over 10 years pharmacist experience as in-patient pharmacist in West Suburban Medical Center, Oak Park, IL, ISJ Hospital, Mankato, MN, and Madelia Community Hospital, Madelia, MN.
American Pharmacists Association Certified Medication Therapy Management Pharmacist, 2011.
Commission of Certification Geriatric Pharmacy certified Geriatric Pharmacist, 2012.

Hi, 
I am Susan,  one of the pioneer pharmacists who devoted my career to Medication Therapy Management Services in the Mankato and surrounding areas.

My belief is that every patient is different, so are their medication regimens, adverse reactions, and treatment plans.

I will spend time with my patients and develop patient-specific action plans for my patients. 

I am also willing to spend time to educate my patients regarding their medications, finding the possible reasons for their adverse effects, trying to prevent those in the first place, and avoiding unnecessary medication usage. The final goal is to help my patients to get the most safe and cost-effective results from their medications.
